Transition from Restrictive Placement/Aftercare

 

Campagna Academy's Transition from Restrictive Placement Program (TRP)/Aftercare is designed for children with behavioral or emotional problems that have begun to transition out of a more secured placement and back into their home/communities. TRP/Aftercare is a court-ordered program for adjudicated youth. This transitional program assists with increasing stability or success in the home, school, or community while preventing a return of youth to a more restrictive setting/placement. We focus on individualized academic needs, emotional adjustment, and family support. TRP/Aftercare services are consistent with the young person's ability, as well as his or her social and emotional functioning levels. 

 

TRP/Aftercare meets the needs of the following circumstances:

n Youth transitioning back to a home after more secured placement

n GED program linkage

n Educational advocacy

n Youth susceptible to negative environmental and social influences

n Individual strength-based treatment plan

n Linkage to community resources

n Individual treatment focusing on adjustment back home, into the

   community, social impairment, and positive decision making

n Employment preparation and opportunities

n Development of social skills

n Enhancement of self-esteem

n Teach personal responsibility to promote successful community living

 

Program Specifics:

n 10 to 18 year-old males and females

n Individual, family support and case management

n Individualized support plan

n Development of pro-social skills

n Vocational, community, and educational linkage
